I think all of us women feel like that, and no it is not a lame-o question, I always feel the same as you do. No matter how much I empty my bladder before exam, soon as that speculum is in, BOOM, I got to go! AND... no you would not be the 1st or last to have any "indiscresion" occur during exam. I will share a personal funny: While I was delivering Miss Priss #2, my OB was checking me as I was pushing, pushing with all my might, I gasped "Did I move her any?" my OB laughed and said, "No, but you emptied your bladder all over my arm!" So yes it does happen!
