Uterine/bladder tenderness during pelvic exam

The last time I saw my gyn, the bi-manual exam was incredibly painful when he pushed on my belly. He seemed to localize the pain to the top of my uterus or bladder. He did a urine dip that was negative and didn't say any more about it. The pain seemed to be in an area that I seem to have an occasional dull ache -- nothing that really bothers me but I notice it in passing and doesn't normally last very long.

What could cause this kind of tenderness? I have my annual in a couple weeks -- should I be concerned if I experience this type of pain again during the exam? Is pain during the exam significant if there's no pain during daily life?
