Is this SOP for Insurance companies?

Since June 1, I have had 6 different health insurance claims for various members of my family. I have had various charges refused on 5 of them and have had to call Aetna to fix. There was no problem with the claim, all they did was re-submit it for processing. I have no idea yet if any of them got paid the second time around. On 2 of these, the item refused was the urine dip ($7) and another was a rapid strep (maybe $15). It almost seems like a deliberate plan to refuse a fairly small charge, figuring the doctor will either write it off or the patient will just pay it. I'm pretty sure it costs them at least that to handle my call to customer service and re-process the claim. I have not had to provide any information or explanation for any of these. There was no reason given for why the charge was rejected in the first place.

Is this becoming common to automatically refuse certain charges the first time around?
