If you have had sex, the hymen is torn. Sometimes the hymen can be torn by inserting tampons too forcefully, but most of the time, this does not happen. You should be honest with your doc about your sexual activity anyway -- if you have had sex, you may need STD screening and a discussion of contraceptive i=options.
