Birth Certificates

This is totally not a medical question, but I figure you folks must sign a LOT of birth certificates and may be able to offer some insight here.

My husband was adopted, but his birth certificate has a doctor's signature on it (dated 11 days after his birth).

Does the doctor sign both the original birth certificate (with the birth mother's info) and the final birth certificate (with the adoptive parents' info)? Or do you just sign the original and the records department transfers your signature to the final birth certificate? Or are adoptive birth certificate signed by a "administrative" doctor who did not actually attend the birth?
