While Bactrim has never been shown to cause birth defects, the two agents in it Sulfa and trimethoprim are both antimetabolites. We don't like to give antimetabolites to pregnant women just because the class of drugs as a whole seems to do some bad things. IOW, there is no evidence that it does harm, but we still worry a little on theoretical grounds. I am unfamiliar with an E. coli infection of the vagina in postmenarchal, premenopausal women. I also don't think that bactrim would have high enough levels in the vagina to do anything. It has high levels in the urine, but not the vagina.
