Without doing the surgery, it's impossible to accurately speculate, but from what you describe it's almost certainly not cancer. It sounds like an inclusion cyst that developed when a small piece of perineal skin was imbedded under the normal skin layer at the time of episiotomy closure. A mobile, round, somewhat firm cyst can develop that may drain once in a while. Excision usually does the trick. In general she should keep the area dry (perhaps by using a blow dryer after she bathes) and use sitz baths (epsom salts). This area usually heals quite nicely. I hope everything turns out well.
