Has she done a home pregnancy test? That will help answer the quesiton about whether this is any pregnancy related complication like miscarriage or ectopic (the latter is pretty doublful if she does not have any pain or if the bleeding is heavy)

Most likely, it is lack of ovulation/hormone imbalance. She may have an ovarian cyst or uterine polyps or fibroids or even an infection. Bottom line -- she needs to see her gynecologist, no ifs, ands, or buts.
