If the fetal crown-rump length is appropriate for gestational age and there is a fetal heart beat with a normal rate (ie 110-160), I really don't see a problem. The sac size in this circumstance for me is not really a big deal. Seeing fetal cardiac activity with a normal rate gives an 85% chance of having a successful pregnancy outcome. Unless there are more findings not related, I don't see as dark of an outcome as your midwife.
