This test is really designed for postmenopausal patients with ovarian cysts. Since it can be elevated with a number of conditions (fibroids, endometriosis, recent abdominal surgery) or situations (smoking), it is not a good screening test in premenopausal women. I know of many women who have undergone unnecessary surgery because their CA-125 level came back elevated. Their gynecologists felt obligated to operate because they were referred with an elevated CA-125 and a small ovarian cyst. It takes guts to ignore that test and proceed with the normal evaluation for a small ovarian cysts these days.
