This is an old-timey term for a "window" or opening in the broad ligament, which is the tissue covering the sides of the uterus. Think of a tight piece of saran wrap covering a bowl. If you take three fingers and slowly push on the saran wrap, it will gradually indent, leaving a concave "window." Most gynecologists think this comes from endometriosis that has eaten it's way into the broad ligament. It leaves an opening behind. By itself this probably does not cause any harm. However, the endometriosis certainly can cause problems.
