The previous thread on this subject concerned a G1 P0 who I was asked to follow because of a compelling family history of preterm birth and "cervical incompetence".

Sure enough, this teenager did demonstrate progressive shortening and funneling of her cervix, and she did deliver at about 34 weeks!

Two lessons: this problem can be "congenital", and can happen with the first pregnancy. Knowing about it, following it, and predicting it does not prevent it.
