You know, Diana, this is absolutely my pet peeve! They are against a TVS "aesthetically"! I've heard this many times, especially from our local hospital sonographers who don't "like" to perform an "invasive" procedure like TVS. It is interesting that they do not consider placement of a Foley catheter to instill water in the bladder as "invasive". Just today, I scanned a lady who had a TAS at the hospital early Sunday morning with a Foley because they don't like for the on-call sonographer to wait while the patient fills her bladder. Top it off, the TAS was inconclusive, radiologist listed all 10 possibilities. Today, when I explained the TVS, she asked why I did it that way. I told her that we considered it to be the most effective type of scan ( I first did a TVS that was also inconclusive, you bet she came in with a full bladder to avoid the Foley). After the scan, which she said was comfortable and not difficult in any way, we were easily able to determine that she had an incomplete abortion.

Perhaps this would not have been so easy to determine Sunday as compared to Wednesday, but I can't help but think that the "aesthetics" of not performing a TVS were skewed in this case so that the patient may not have received the most appropriate care. Personally, I would much rather have a probe placed in my vagina than a Foley catheter in my urethra! I'll get off my soapbox now. You do understand that I am venting and this post is not directed to you in any way as a flame. We seem to be on the same wavelength about this.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
