I think you make good points. Our formal program uses Web based clinical records, time sheets, and case logs... all with password encryption. I also agree that, "I am not sure that it is the best model." But as a good friend, George Ellis of Cape Town, told me, "Examinations are not the best way to determine if students are learning, but many won't read if you don't test." Over the years we have had to add more rigor to the rules because a few want to fudge the limits of our program's requirements. When it comes to passing a national professional examination in order to get employment, then the limits have to be well defined because there are those who don't care about the means as much as the end job. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
