The gestational sac of the fetus appears to have a normal placenta, so my guess would be either a dizygotic twin gestation (one normal and one molar pregnancy), or a pregnancy and, given the absence of much normal myometrium, coexistent invasive mole. Given the patient's history and the appearance of the myometrium, I think the second scenario is the more likely possibility. I don't have the clinical background to offer an opinion on the outcome of the pregnancy or the management of the patient. Paula
