I think this represents a uterine synehia (very common) with stricture (less common) rather than dehiscence of old uterine scar (very rare), for following reasons.

1. Associated with distortion of uterine cavity, with uterus which seems to continue around.

2. Does not appear to affect fetus or membranes otherwise (needs to be confirmed with more views).

3. No anomalies seen - at least on these views.

This (assuming myometrium actually surrounds the area & that rest of fetal scan is actually normal - and this is a stricture rather than an opening in the uterus) is low risk as placental implantation is not involved.

I had a patient with one of these who felt a "pop" and had vaginal bleeding, preterm labor and ROM associated with one of these that broke that involved the placenta.

I currently have one right now with a similar appearance except that the placenta is implanted on both sides and is quite vascular. She is at risk for antepartum and postpartum hemorrhage from this.

It depends upon your ISP and email reader/editor. Several here cannot access the images.
