Re: Informal Case: Nuchal Translucency versus Cystic Hygroma

Y'all:

I do not believe these data. In our own practice, the risk for hydrops, preterm, cardiac or chromosomal anomalies is over 40 percent when the NT is this thick, although we have plenty of healthy liveborns after these findings. Saw two last month, one with 4mm and one with 6mm.

Reinders, Paulker & Benacerraf J Ultrasound Med. 1997 Feb;16(2):101-5) had a similar experience. Using a standard 3mm cut-off and nuchal lucency as the ONLY finding on us, 27 of 41, or 2/3, had a normal outcome (tho 12% were karyotypically abnormal). It does distress me that 3 of 44 aborted without any further investigation.

It would lead me to suspect that they have measured the fetal - amnion distance.

80% of cases with NT between 4.5 - 6.4 mm result in HEALTHY Liveborn Infants. Souka et al. Ultrasound in Obstetrics and Gynecology 1998;11: 391-400.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
