Joanne,

A D and C can be performed under epidural anesthetic and also under local anesthetic called cervical block. As far as mother nature taking its course on retained products of conception,that depends on the condition of the patient (temp, hemoglobin-hematocrit level and her resumption of menstration). There have been cases of pregnancies associated with dead fetues not expelled naturally only to be detected later in life - so nature does not always takes its course.
