It looks like this fluid is in the endometrial cavity. I do not think it so unusual to have some fluid still in the cavity after a delivery. Consider the possibility of a cervical stenosis, or, since this was a C section, the remote possibility that the anterior and posterior aspects of the cervix were sewn together during the closure of the uterine incision (yes, it has happened.) Why don't you scan her again in 6-8 weeks.

I just received this case today, never saw anything like that. A c-section was performed on jan/6 for placenta abruptio on this patient. On 18th she started a monthly injectable contraception (Perlutan: a combination of estrogenel+progestine). She is complaining of dizzeness and irregular bleeding. On USTV she has this image on the fundus of the uterus. Physical exam is unremarkable.

Any idea?
