There was a coding error that caused many of you to get an error when you submitted your survey and we did not get your response. Error occurred if contact information fields like address or zip/postal were left empty.

If this happened to you and if it isn't too much bother please resubmit the survey one more time so that we get your responses and so that you are entered in the drawing for a AIUM or COGI registration.
