endometrial mass

43 y/o multiparous, obese pt with enlarged uterus(20 wks), progressively heavier and longer periods x1 yr, with lumbar and bilateral hip pain. No prior h/o gyn or fertility probs (except pds of anovulation secondary to hypothyroidism). Pelvic ultrasound shows well-defined rounded cystic mass (9 x 8.6 x 9.4 cm) posterior to uterus with multiple low-level internal echoes. Radiologist says most likely secondary to endometriosis (no prior hx)...what other pathology could be indicated?
