In my opinion, intraabdominal umbilical vein aneurysms or varices are not uncommon. We see them about every six months, most of them small. I have never seen adverse outcome with these or associated anomalies, although in older studies it seemed to worry the sonologists and some reports of adverse outcomes have been reported. Extremely rarely, intraluminal thrombus can be seen in the dilated umbilical vein.

Normal umbilical vein measures 3mm in diametre at 15 weeks and steadily increases in diameter in a linear fashion to 8mm at 36+ weeks.

I think bidirectional flow pattern on spectral doppler is very likely due to swirling of the venous flow in the dilated portion of the vein, much like the ying-yang flow seen in AAA.
