If there is no funnel, the length of the cervix is from the internal os to the external os. If there is a funnel, measure the length from the apex of the funnel to the external os. Again, be very careful that your transducer is not influencing the length of the cervix. If the cervical length is not clearly normal, or you have reason to believe that the cervix may be short, funneled, incompetent, etc, it may help to ask the patient to stand up, while you keep the transducer in her vagina. Wait about 5 minutes, then reevaluate the cervix, being careful not to compress the cervix with your transducer. Others press on the fundus to stress the cervix. I have not had luck with that. Perhaps I do not press hard enough. I am not sure that you can say 3 cm or more is normal, less than three cm is abnormal. I am sorry I cannot give you the references, but you should be able to find them in Medline, or even in Google.
