Dear OBGYN.net Ultrasound Community,

Nordeval Cavalcante Araujo of Brazil has submitted the following case for your review: http://www.obgyn.net/us/present/0102/nordeval1.htm <http://www.obgyn.net/us/present/0102/nordeval1.htm>

Could this be a case of polycystic kidney disease?... See our editorial advisor's viewpoints below.

Yours, Martin Necas OBGYN.net Ultrasound Section Coordinator martin.necas@obgyn.net

"I would guess autosomal recessive (infantile) PCKD. The author did not indicate whether measurements were taken, but the kidney(s) appears very large relative to fetal body size. I am surprised a little at the amnount of AF, but it's relatively early on in pregnancy so I guess if it is PCKD the AF will reduce during the second trimester." -Martin Necas

"Yes, I agree" -Allen Worrall

"I would also go with IPRD. The reduction in liquor volume will not be apparent at this gestation as at 15 weeks liquor is mostly transdudate. I have a very similar case ongoing at the moment with huge kidneys" -Ross Welch

"the images do not conclusively show, whether the echgenich kidneys are bilateral. The differential diagnosis is among others ARPKD and ADPKD. In the later case one of the parents should have a similar problem. Therefore US of parental kidneys and or measurement of parental blood pressure should be done. In the case of suspected ADPKD there is noot necessarily a reduction in AF with ongoing pregnancy. Prognosis is "similar" to that of the affected parent. I would be interested to hear about the outcome." - Josef Wisser
