Andrea, go to the following URL and submit (upload) the images from there... one at a time. http://www.obgyn.net/scripts/dbml.dll?template=survey&v=9 Just a couple of hints. First do not hit "Enter" as you finish each information item, either tab or us your mouse and cursor to move down the list. If you hit "Enter" then you will submit the form incomplete. Use the "OK-send this form" button at the bottom to submit. To submit an image, please make it relatively small, say 10-12 cm wide or so... use JPG or GIF file formats for smaller file sizes. No BMP, please, they are huge. Once you have the image on your hard drive, then use the "Browse" button for the "Image for Upload" to locate it and double click on it to put the name in the form. That saves typing the path and file name and the typos that keeps the upload from succeeding. Then if you have more than one image to submit, especially from the same case (same information), then after you send each image file, use your back button and only change the "Image for Upload" file name.
