Re: Single ultrasound

The patients bring the tapes, we just fill them. I agree about the lovinox & have not put anyone on that drug for pregnancy.

I did dopplers when there were no codes, no reimbursement and good indications. We have and will do cervix checks without reimbursement with no dx, cause I say so and it saves babies. BTW Hats off to JT Christmas for actually getting the MFM codes to reimburse for targeted scans.

The former costs $2500 per month but has become the de facto standard of care. The latter is $78 per month. At term we have to switch back to heparin because of the shorter wash-out and possibility of epidural anesthesia. Is this indicated? WHo should pay the $2422 difference in cost?

The rational indications for ultrasonography when the cost to the patient is zero may be different than that when the cost is full UCR price, and will be different for different patients for different reasons. The "my way or the highway" approach to medicine is wrong in a free society, IMHO, but then I'm the guy who tapes on demand. About 1 in 4 patients actually NEEDS a sonogram for a medical problem in pregnancy... Unfortunately we often donot know which one of the four unless we do the scan....

if you want to give out tapes hey ... ok by me ... if you feel a course of action is medically right and the patient agrees, but insurance doesn't cover ( this week), would you not do what you think is right? btw lovenox during pregnancy is controversial, no?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
