I need some opinions and input. My research so far has been fruitless, so I am hoping that some of you will have had personal experience with the following topic and can offer me some guidance. My question is regarding the possibility of decreased absorption of COCs after gastric bypass surgery. Is this something to be concerned about? Theoretically, it certainly seems very possible. Myself, I have encountered a couple of bypass patients recently - one who had been using Tricyclen for years without issues, but started having BTB (suggesting to me a decreased E2 absorption) after her bypass and presented in the office concerned about OC effectiveness; the second patient got pregnant fairly quickly after her bypass surgery despite excellent use of OCs and is now using DMPA successfully. What is the general opinion on this topic? If OC absorption is decreased to subtherapuetic levels, what are the alternatives? A 50 mcg pill? Injectables (DMPA or Lunelle) or patches? Is anyone aware of any reputable studies that concern this issue? Thank you so much for your wisdom and input!
