burst uterus during delivery

A friend of mine just had a horrible experience giving birth. She was in labor for over 30 hours and her uterus burst. Apparently the baby was without oxygen for too long and is now in critical condition with severe neurological damage. My question is, what causes something like this? Are there signs to know it may happen? and How can you prevent it from happening?

I ask because I'm currently 20 weeks pregnant and all I can think about is what she's going through.

Thanks, Tracy
