Pregnancy after 40

I've had three children - After the third birth I had my "tubes tied" 15 years ago - Sorry don't know the official medical term for this procedure. My fiancee and I want to have a child - problem: I am 42 years old now and want to know if the procedure could be reversed or whether I need to resort to some other method of getting pregnant (in-vitro?).

Also, if I did have the surgery, would it be covered by insurance and if not - how much does it cost approximately?

Thank you.

Marcia
