alcohol, surgery and birth control pills during pregnancy

I took a pregnancy test a month ago and found out I was pregnant. I was living on a boat on some very remote islands at the time and was not able to get to a doctor until this week. She estimated me at being 19-20 weeks pregnant. Needless to say, I was shocked. I have several concerns that someone can hopefully respond to...

(1) 12 weeks ago I had breast aug surgery and was put under with general anesthesia. I took pain pills and antibiotics for 10 days following surgery. My doctor said they were doing a pregnancy test prior to surgery. What happened? Why didn't it show positive? I haven't yet discussed this with my breast aug surgeon as I am waiting to have a level 2 ultrasound next week.

(2) I was taking birth control pills up until 4 weeks ago. What effects can this have on the baby?

(3) I was drinking regularly up until a month ago(averaged probably 3 drinks 3 times a week(wine 2x and liquor 1x). I went on vacation 16 weeks ago and drank rather heavily for 3 days(maybe 6-7 liquor drinks each day).

My doctor has scheduled me for a level 2 ultrasound, but I can't get in until next week. She was not deeply concerned and told me to not worry about anything until I have the u/s. She said she was sending me for a level 2 basically for my peace of mind. In the meantime, I am very concerned. Please provide any information you may have on the above issues. Thanks.
