I am no fan of episiotomies, and I probably only make 1-2 per year. Nevertheless, the 2 that I have done this year both saved the life of the baby. When you draw a line in the sand, just remember that you are using your baby as the stakes for the wager. Never say never. It is much better to discuss your reservations about episiotomies with your provider, discover his/her philosophy about them, express your profound desire to avoid one, and so on. Whenever a patient tells me that they don't (or do) want something "no matter what", I ask them to write in their chart that they would rather die and allow their baby to die rather than have the intervention in question. This is most common for Jehovah's Witnesses, who would rather die than receive blood transfusions. Their religious beliefs come first, and I can respect that, even if I don't share those beliefs. Similarly, I can respect your beliefs to avoid an episiotomy "no matter what", just so long as you realize that 1/1000 times, the "matter what" may be a catastrophe.
