I can speak to the difference in practioners. I have had 4 children with 4 different practitioners. I think there is a definate difference between them. Some are plainly "better". I don't know if it is a matter of patience, skill or what. I had a speculum exam during labor with #4 and it was NO BIG DEAL. I've had more uncomfortable exams by other practitioners for a routine PAP smear. Also, my cervical checks during labor were practically painless this time. I think because I was not checked during a cx. With my third child I was checked during cx's and those were the only times I completely flipped out and lost control. Maybe internals ARE more painful during pregnancy for some women, I don't know, but in my experience not all practitioners are equal when it comes to the comfort department.
