I had a midwife who was there for ME(CNM in hospital), a great L&D nurse who I also didn't have to share and my husband. If you are birthing in a large institution where you will probably be left alone with hubby most of the time with only occasional visits from a nurse and an OB who flies in to catch the baby then a doula might be a good idea if you can afford it. I know if I didn't have the support system I had I would definately want a doula. Karen
