Can it be preterm labor without cervical change?

I am currently 30+ weeks pregnant w/ my 2nd child. My first child was born at term 12+ years ago. That pregnancy was pretty normal except for 50-80% effacement starting at 16 weeks. I was monitored throughout and there was no dilation and I ended up being induced at 41 wks. This pregnancy has been a totally different ball game. I started bleeding at 12.5 weeks. At that time my cervix was at 2.5 cm. The bleeding became progressively worse and at 14.5 weeks I was diagnosed (by a perinatologist) with a moderate marginal placental abruption. I was put on bedrest and at 18 wks the abruption had partially reattached, my cervix was up to 4.5cm and I was released back to an OB.

I have noticed Braxton Hicks contractions since the abruption. They have become for frequent and more intense. I am now at the point where I cannot do any physical activity without inducing very frequent contractions (~every 5 min). A vaginal u/s at 23 weeks showed no funneling and my cervix at 4.5 cm. My OB doesn't seem concerned since there is no apparent cervical change, although I have not been checked since 23 wks.

Here is an example of the frequency of the contractions I notice because of the discomfort they cause: 12:53, 1:16, 1:37, 1:42, 1:51, 1:59, & 2:11 These all ocurred while I was sitting quietly.

Since I have a history of an abruption, I understand I am at higher risk for preterm labor due to PROM. Am I at risk in any other way? How do I know if I am in PTL without going into the dr. for a cervical check? Are these contractions really nothing to take seriously as long as there is no cervical change? Should we be doing SalEst or fFN tests? (I live 3 hrs from the OB/Hospital) I am starting to get nervous about all these contractions as they are increasing in frequency.

Thanks, Alison
