Prenatal concerns

Yesterday, I had a vaginal ultrasound to confirm fetal age. I am 9 weeks along. We saw the fetus and the heartbeat. My doctor said that development of the fetus itself looks right but that there was an obvious absence of the yolk sac, and what she called "changes in the placenta". What does it mean? Is this unusual?

I am 31 years old and I have Type I DM, diagnosed 6/98, and now under control via insulin.
