death of baby following pitocin induced labor

On September 3, 1998 my labor was induced at 40 weeks due to the baby getting big for my size (I'm 5 feet). I was having minor contractions about 4 minutes apart and was not dilated at all. I was given pitocin and the dosage was increased because the contractions were not getting much stronger. After 8 hours, the contractions were strong and I was fully effaced, but not dilated. The nurse broke my water at that time. In the next hour, the contractions were one on top of the other and I dilated to 10 cm. I began pushing. The fetal monitor was showing decels during the contractions and they had me breathe through a couple contractions without pushing. I was given an oxygen mask and continued pushing. After 30 minutes, they were going to use a vaccuum extractor for assistance. They took the fetal monitor off her head and my baby was born within 1 minute. She never took a breath and they never detected a fetal heartrate after her birth. She was 8 lbs 4 oz and 20.5 inches long. An autopsy report showed no problems or cause of death.

My question is, should anything have been done differently and was it the pitocin that caused the fetal distress? My doctor and I agree that we should have a C-section next time - is this appropriate?
