The APA antibodies are very variable. I look after many women per year with this problem and the results vary considerably from week to week and are often only positive in pregnancy. Different labs get different results on the same blood sample! OUr labs break the tests into anticardiolipin (subtypes) lupus anticoagulant and also test other rare causes such as Protein C, S activated protein C and where the above results indicate they look for factor 5 Leiden mutations. Different treatment regimes are indicated for different problems and your specialist will be able to guide you through the maze. There was a large series published in the British Medical Journal earlier this year from Prof Lesley Regan's unit in London. Her conclusions were that aspirin and heparin (5000 iu once or twice daily) was best in outcome terms. Many of us use "Fragmin" (a low molecular weight heparin) as it is a once a day treatment and has been shown to be associated with considerably lower loss in bone density during pregnancy than calcium heparin. I would suggest you go and have a long chat with your specialist before commencing another pregnancy and get a plan in place. Don't forget the folic acid from before conception until 14 weeks as well. Best of luck.
