6 cm dilated & 80% effaced at 36 weeks, not in active labor

I am 36w/1d with my second baby and am 6 cm dilated, 80% effaced and the baby is at -1. One week ago, I was 4 cm dilated and 60% effaced. Is there any way to tell how close I am to delivering my baby? I have frequent contractions, often painful, but they are not organized: ranging from 4-6 min apart to random painful contractions mixed with milder ones throughout the day.

I am concerned about knowing when to go to the hospital. At what point should my water break? Is it related to my effacement more than the dilation?

Are there any risks to my baby by walking around in such an advanced stage and not be in active labor? I am a believer in natural child birth, but wonder at what point I should consider asking my ob/gyn to break my water to bring some relief to the situation and deliver the baby. I delivered my first baby very quickly: within 3 hours of my water breaking spontaneously. Should I be concerned about getting to the hospital in time since I am so advanced?

This situation has me baffled and I have not been able to find another story like mine yet. Any help would be much appreciated!
