If the labor is in a hospital environment with capacity to make a c-section quickly enough then, I think you can try a vaginal delivery

Yhe same for this question, depends on where the delivery is going to take place. And finally if you are not sure enough, go for what you and your doctor feel. Talk to him, ask him this questions that you have and listen to his opinion (that is, I think, the wisest way to do this)
