On a Level 2 ultrasound - where a specialist performs it, the signs (markers) of downs are caught more like 60%-70% of the time. Additionally, there are some markers that are much more definitive of ds. (nuchal fold and nasal bone) A second L2 u/s done 3 weeks later will catch additional markers. So, it's not foolproof, but only you and your perinatalogist can decide that. Amnio is a sure fire way to know, but given the miscarriage rate, may be more risky than the risk of your baby even having downs syndrome. On an u/s in a dr's. office, I have no idea of the accuracy. I've read studies, none site very high results. The dr's are not specialists in that field, although, I'm sure some are better than others.
