I don't know about test sensitivity changing within the past 15 years but let me share a recent experience with you. My friend took numerous HPTs the first five weeks after her pregnancy and they all came back negative. She had never been pregnant before but just "knew" she was. She went to the doc who performed blood work, came back negative. They did a round of bloodwork for an additional 3 weeks before finally getting a positive result. Unfortunately her pregnancy ended at 9 weeks in miscarriage. Reason: hormonal imbalance. Which is why the tests kept coming up negative. I don't know if you are experiencing negative results, which is why you expect a miscarriage, or if you've had bleeding or what. Normally, if one does miscarry, the hormones remain in your system for a while so tests would still show positive. As for the ultrasound, at 4 weeks, you may or may not see a sac. Fetus and heartbeat usually aren't seen until around 6, maybe 7 weeks. Good luck with everything! Let us know what happens!
