Sometimes nothing can be done. I had a regular episiotomoy but b/c my baby was posterior, they had to use forceps and I tore further through my muscle, level three. You can try perineal massage next time, that is where your DH or the doc or a nurse/midwife massages the perineal area with a warm, wet cloth to relax it and let you stretch. My DH did that with me and it would have worked had my son not decided to come out facing sunny side up;) Also it helps to push gradually so the doc can gently stretch you around the baby's head. Although, be comforted by the fact that the 2nd birth for most women is shorter and easier. Sitz baths are wonderful, also the hospital gave me a lidocaine spray to use, that helped me alot.
