Just my thoughts if I were in your position:

1) You're gonna need a REALLY supportive doctor. Find one now if you don't already have one. It may take a lot of time/interviews. Make sure you discuss potential problems and complications both with getting pregnant and having a healthy child. Make sure that you're not risking your life (and more importantly the life of your children's mother).

2) Join a gym or exercise group and see a nutritionist/dietician/doctor. I would make it a goal to get all the way into a normal weight range and to see if you can lower your bad cholesterol thru diet and raise your good cholesterol thru exercise. Consider yoga classes or something similare to increase your lung capacity without aggravating your asthma. Be ready to go back on the inhalers (especially initially) if you need them to get thru an exercise program. Many asthmatics simply breathe shallow and more often in order to oxygenate and any deep breathing aggravates their symptoms.

3) Get your kids/spouse involved. Make them aware of your goals. Discuss any changes in diet/nutriton with them and why you need to do this and how eating healthy helps them to. Walk, jog, or ride bikes with them (or let them ride while you walk/jog)

4) Put a calendar on the refrigerator and record all physical activity and your weight each day. Have a realistic goal for each week/month. Shift the goal as needed and make the entire procedure into a giant game for the whole family.

You may have rapid initial weight loss the first week or so due to diet/exercise changes but after that initial shift you should only lose a couple of pounds per week. Plan on it taking 6-8 months.

If you are unable to get pregnant or to safely be pregnant please look into foster parenting.
