4th IUI and just learned I didn't release my follicles?

I have had three previous pregnancies .. one ending in a healthy child. I had no problems getting pregnant with her.

My doctor after the IUI asked me if we had ever checked by a sonogram to see if I am releasing the eggs and I said NO so I went in today and they had not released and it has been more than 48 hours after the shot so I do not understand what the deal is...

1. is this a fluke 2. did I not take enough HCG shot 3. why would this happen when I have released before
