I am a ENT nurse and would agree with the doctor that little absorption occurs from the ear canal. It is a catagory C drug due to lack of studies and is not used in children under the age of 18 because it can effect bone growth. If it is worrysome to you as it is to most pregnant women I would just call the doctor back and ask him to order you some gentamycin otic drops. As far as the AFP level it is considered elevated or positive at 2.5 m.o.m, so .78 is good.
