confusion over pregnancy dates

I used the following site for the calculations:

I hope this isn't too confusing to follow. My cousin has two children. The LMP of her first pregnancy was on 10/19/99. On an ultrasound picture she has from 4/17/00 for that pregnancy, it says 21W1D+-10. We know that ultrasound goes by gestational age and not by the LMP, but even so, there seems to be a discrepancy of about 3 weeks between her dates and the ultrasound dates.

The LMP of her second pregnancy was 5/06/01. The ultrasound picture from that pregnancy (taken on 10/10/01) states that the age is either 17W6D+-6 or 18W6D+-6 (it lists both on the same picture). Again, there seems to be a discrepancy of about 3 weeks with that pregnancy as well.

What could be the cause of these discrepancies? She is certain of her LMPs. How would she go about calculating the dating of a possible pregnancy with such a discrepancy? She feels she may be pregnant again, but the last few tests have been negative. She has had many symptoms and they persist even after the negative tests.
