19w - spotting after internal exam - dr. please

I am currently 19w, and today I had an internal exam to check the cervix, due to numerous painless contractions and some lower back and abdominal pain over the last week. The nurse practitioner who did the exam said the cervix appeared long, firm, etc. (all good, I guess), and everything else seems normal -- heartbeat, size of uterus, etc.

The problem is that now I'm spotting, brownish blood with some mucous, not too much, but still... The nurse said I would b/c she saw some blood on the glove after the exam. So it's not unexpected, but it still make me nervous. Can an internal exam at this point in the pregnancy cause problems -- infection, tearing, anything? How about if the placenta's really low and we don't know it (my ultrasound's not for another 3 weeks)? Sorry to be so paranoid, but this is my first pregnancy and everything seems to freak me out :-)

And how long can this spotting continue if it's normal?
