It seems like all doctors and all insurance are different. . . My doctor doesn't do routine first tri u/s because insurance generally won't pay. He will do them if you're spotting, cramping, experience a decrease in symptoms or have no clue when your lmp was. I had one both pregnancies (the first because my symptoms had lessened and the second because of bleeding--everything was just fine both times!). 6 weeks is right on the borderline of when you're able to see a heartbeat, so don't freak out if you have an u/s and don't see one yet. Both mine were after 8 weeks so that wasn't an issue. I got pictures both times. Also, my u/s were transvaginal, and I was told to EMPTY my bladder. Good luck.
