I am 24 weeks pregnant. Due to recurrent uti, I was placed on Macrobid prophylactiaclly 100mg hs. I had a sterile catch urine last Tuesday at the Urologist, and it showed no growth. This Monday my OB did a clean catch and it came back with enterococcus 10-50,000 colonies. He said that it was sensitive to Macrobid and that i should take it B.i.d. for 3 days and then go back to my once a day regimen. I also had a vaginal swab done 2 weeks ago, and it showed no strep b. When I asked about bv, he said that the culture was non-specific and it would have picked up any infection. So, here are my questions: 1) I realize that 10,000 -50,000 isn't a major infection, but if I'm on Macrobid prophylactically, shouldn't that have prevented it? And will taking it b.i.d take it away since the culture showed suscepibility to it? 2) If I had a sterile catch that showed no growth last Tuesday, does this sound like it's a vaginal infection vs. a urinary one? And, if so, is that dangerous for PROM? 3) Should I be doing any other vaginal tests than what he did? 4) Since Macrobid is category D in the third trimester, and cephalosporins can cause an overgrowth of enterococcus, would you recommend amoxicillin if it is suscepitble to it? Thank you for all of your help. I am just paranoid about pre-term and thought I did everything I could by taking this prophylactically and doing some sterile catches.
