2nd trimester losses

I was wondering what are the most common causes for 2nd trimester losses?

People tell me now that I passed my 12 weeks, that I should be confident, but I keep hearing stories about losses at 14 and 16 weeks.

I have had 3 earlier losses, so that is the reason for my paranoia.
